PAYMENT INFO

Cash or Credit Cards are accepted for co-payments. Cash for other services rendered will be accepted with prior approval. Please ask to speak with the Office Manager.

HEALTH INSURANCE
Dr.Wong participates with most insurance plans and will submit claims to all health insurance plans. It is your responsibility to determine if Dr. Wong is a contracted provider within your network. You will need to contact your insurance company to determine what services are covered and if a referral is necessary.

Please bring your insurance card with you to your office visits. A copy of your card will be made. If your insurance requires a referral, please ensure your primary care physician has issued one prior to your appointment. If you are required to have a referral, but do not have one, you will be financially responsible for the visit OR rescheduled.

Currently, co-payments will be billed after each visit.

WORKERS COMPENSATION
If you have been injured on the job, Dr.Wong may be available to provide your care. Workers Compensation patients are accepted on a case-by-case basis. If you want your visit covered by Workers Compensation, then please ensure that you have the following information prior to contacting the office:

  • the claim number
  • the name AND phone number of your adjuster
  • the name AND address of the insurance company

MOTOR VEHICLE ACCIDENTS
If you are involved in a motor vehicle accident, Dr.Wong is available to provide your care. Each visit must be paid for at the time of the visit. Dr. Wong does not accept liens, does not bill automobile insurance companies, and does not bill Attorneys. Should you choose to pay out of pocket we will provide receipts for you to submit to the appropriate entities. If you choose to use your own health insurance, please obtain a referral from your primary physician prior to your scheduled appointment if necessary.

UNINSURED PATIENTS
Full payment is required at the time of service. As above, cash for services rendered will be accepted with prior approval. Please ask to speak with the Office Manager.

OUTSTANDING ACCOUNTS
Dr.Wong expects payment for services rendered. He also understands that circumstances arise and that times may be tough. He is more than willing to set-up payment plans if necessary. Should you disregard bills and / or fail to negotiate a reasonable payment plan, you WILL be sent to collections. This may have a negative effect on your existing credit, your ability to obtain future credit and / or future employment.
